Clinic closure

By now, you should have received a letter from me informing you of the impending closure of my clinic, due to my accepting a position at a U.S. naval hospital overseas. If you haven’t, look no further. I’ve reproduced the letter here.

Please also note that the closure of my practice means that I will no longer be a contracted provider for any insurance companies with whom I had previously contracted.

Most importantly, I will miss all of you. These have been the happiest years of my life thus far.

Dear [insert name],

I recently learned that I have been selected to be the chiropractor at a U.S. naval hospital. Because of the overseas location, I am obliged to relocate and will therefore close my office at 6887 Brockton Ave., Riverside, CA 92506 on 30 November 2019. I will not be available to attend to you after that date.

Since your condition may require additional care, I suggest that you arrange to place yourself under the care of another doctor. You may contact your insurance company or conduct an Internet search, but I would like to refer you to Renee Cauvin, DC. Dr. Cauvin operates a mobile practice, which means that she will meet you at your residence or workplace to provide you chiropractic care. Because of this additional convenience, Dr. Cauvin does not accept insurance. You can contact her at 512-947-1251 or docreneedc@gmail.com to make appointments. She is also an animal chiropractor and can provide such care for your four-legged friends.

To assist you in transitioning from one chiropractor to another and in receiving the care that you need, I will be happy to provide you with copies of your records. Please email me requests for records at wx2d@virginia.edu. This will be the primary and most reliable method of contacting me during and after my relocation.

I regret that I will not be able to continue to serve you. My years here have been filled with many rewarding experiences and memorable patients. You can continue to follow my adventures abroad at my website, xiachiropractic.org.
